Background
The Institute of Public Policy is working with Dr. Julie Kapp, Associate Professor with the MU School of Medicine, to develop and deliver educational training on enhanced care coordination for providers working in areas where needs are high, and resources are limited. One of the initial steps in that effort was understanding and describing the current state of care coordination in rural Missouri, especially those related to addressing social determinants of health.
Health care providers increasingly turn to care coordination to address social needs and other non-medical determinants of health. Care coordination may be particularly valuable in rural areas given health inequities. Yet, research into the structure and implementation of rural care coordination is limited.
Next Steps
Our next steps will be the development of a manuscript detailing these findings and the development of a web-based curriculum that synthesizes the state of the science on care coordination integrating social services with available resources and best practices.
